NURSING HEALTH HISTORY

PRESENT

Three days before the hospitalization the patient suffers from headache, body pain, “on
and off fever”, diarrhea, nose bleeding, and rashes all over her body. Her family
performed tepid sponge bath and gave the patient an over the counter drug which is
paracetamol, as medication. After three days of suffering the parents of the patient
decided to bring her immediately to the hospital.

PAST HISTORY

According to the patient, she has past history of dengue three years ago. She suffered
the same symptoms before. The patient was admitted to the hospital three months ago
due to Urinary Tract Infection.

SOCIAL HEALTH HISTORY

The patient is in 10th grade and studying at La Salette of Ramon, INC. She walks going
to school as early as 6:30 am and according to the patient they need to clean their
classrooms and its surroundings before they go back home around 4:20 pm. According
also to the patient, she helps her sister in doing household chores such as washing the
dishes, sweeping the floor, and watering the plants every weekend. She lives with her
older sister and her mother because her father is an OFW in Taiwan.

FAMILY HISTORY

According to the patient’s mother and the patient, itself, their family has no history of
any hereditary diseases and has no past history of dengue except the patient, itself.
