ASSESSMENT OF THE ABDOMEN RATING
ASSESSMENT SKILL
1. Perform hand-washing.
2. Gather equipment (pillow/towel, centimeter ruler, stethoscope, marking
pen)
3. Explain the procedure to the client.
4. Ask the patient to wear a gown.
5. Inspect the skin, noting color, vascularity striae, scars, lesions (wear
gloves to inspect lesions).
6. Inspect the umbilicus noting color, location and contour.
7. Inspect the contour of the abdomen.
8. Inspect the symmetry of the abdomen.
9. Inspect the abdominal movement, noting respiratory movement, aortic
pulsation, and/or peristaltic waves.
10. Auscultate the bowel sounds, noting intensity, pitch and frequency.
11. Auscultate for vascular and friction rubs.
12. Percuss the abdomen for tone.
13. Percuss the liver.
14. Percuss the spleen.
15. Perform blunt percussion on the liver and the kidneys.
16. Perform light palpation, noting tenderness or guarding on all quadrants.
17. Perform deep palpation, noting tenderness or masses in all quadrants.
18. Palpate the umbilicus.
19. Palpate the aorta.
20. Palpate the liver, noting consistency and tenderness.
21. Palpate the spleen, noting consistency and tenderness.
22. Palpate the kidneys.
23. Palpate the urinary bladder.
24. Perform the test for shifting dullness.
25. Perform the fluid test.
26. Perform the ballottement test.
27. Perform the test for appendicitis:
a. Rebound tenderness
b. Rovsing sign
c. Reffered rebound tenderness
d. Psoas sign
e. Obturator sign
f. Hypersensitivity test
28. Perform the test for cholecystitis:
 Murphy sign
