I concur with your view that NASA is to blame for the Columbia Space shuttle accident

that killed seven astronauts. NASA exhibited negligence and incompetence by failing to call off

the mission despite noticing mechanical issues with the space shuttle shortly after departure.

Besides, NASA did not learn from the prior incident that involved the Challenger. These two

cases prove that NASA was not ready to incorporate changes into their shuttle safety protocols.

However, I differ with your stance that resistance to change is caused by ignorance and pride.

Although these two might play a role, there are other factors such as financial restrictions and

fear of the unknown that can make organizations resist change.

Security cautiously to avert adverse outcomes. I support your suggestion of being cautious with

the type of volunteers admitted. However, caution alone is not enough. The department needs to

outline precise qualities and qualifications that aspiring volunteers should possess.
