2. Work Location : Sidoarjo, Jawa Timur
3. Client will provide:
i. All letter addressed to Migas from the owner of equipment.
ii. Permit to work in Site and Fabrication Area.
iii. All required technical document for inspection and Certification.
iv. Ijin Usaha dari owner
4. Price include for:
i. Inspection Activities : Review, Verification and Witnessing for Hydrostatic Test
for Pipe line and Popping test for PSV unit.
ii. Meal, transportation and accommodation of our inspector, & Migas personnel
during site inspection.
iii. Migas arrangement for inspection & certification
iv. Qualified inspector with APD and valid MCU.
v. Inspection report, PLO & COI issuing.
5. Price Exclude :
i. Peneleaahan Desain untuk PLO (By Engineering Company)
ii. Calibration for Meter dispenser.
6. Partial order is not applicable.
7. Term of Payment Inspection & Certification Services:

a. 50% DP after PO signed by PT Valarbi


b. 50% Final Payment After COI & Report.
c. All invoice paid in maximum 30 days calendar.
d. Mobilization will be conducted after DP receved by PT Valarbi.
8. PO Cancelation will be charged 25% of total
9. Quotation validity: 30 days.
