LESSON LEARNED

1. Some Piping spool welded during pre-fab were detected to have been wrongly fabricated/wrong
dimensions during installation because Pre-weld dimension control check was not done due to
concession requested by PMT in other to expedite project delivery

Solution:

 Pre-weld dimension check is required on all structural, piping and vessels works. Removal of
inspection requirements usually leads to non-conforming products and the attendant reworks and
costs. Approved inspection points as per ITP should be implemented from fabrication until
installation and testing.

2. Interface between engineering and construction did not promptly capture changes made on site.
Design change register was not up-to-date due to lack of proper harmonization of changes made

Solution:
 Interface should be tasked with the responsibility of giving account for all design changes made
on site
 Construction to notify the essential persons before noted changes can be carried out on site.

 Dedicated personnel should be assigned the task of releasing all redlines promptly within
Engineering team..

3. Construction team carrying out site modification of structural, piping and vessel works without
prior notification

Solution:
 Fabrication/Construction Front Line Supervisors should always respect the ITP in carrying out
modification/re-works
QA/QC should be notified prior to commencement of reworks so that the client can be notified as
per ITP and all QA/QC records followed up appropriately
