AFFIDAVIT
(Parental/Legal Guardian’s Consent)

I/We, ________________________ and _________________________, (both)

of legal age, married/single, Filipino(s) and resident(s) of
_____________________________________________________________ and
_____________________________________________________, respectively, after
having been duly sworn to in accordance with law, hereby depose and state:

1. That I/we are the parent(s) and/or lawful guardian(s) of

______________________________ who was born on ____________________ at
______________________________________________________________;

2. That I/we am/are allowing my/our son/daughter/ward

_________________________________________ to undergo military training at
Philippine Military Academy (PMA);

3. That I/we am/are allowing my son/daughter/ward

____________________________________________ to be commissioned as a
Second (2nd) Lieutenant/Ensign in any of the military service branch (Philippine Army,
Philippine Air Force and Philippine Navy) of the Regular Force of the Armed Forces of
the Philippines (AFP) where he/she may qualify upon graduation from PMA;

4. That I/we obligate myself/our self to be held solidarily (jointly and

severally) liable with ___________________________ in the reimbursement/

payment of all the expenses incurred by the government in his/her entire period of
training and stay with PMA in the event that he/she shall voluntarily resign as a cadet
for any reason. Further, in the event that he/she willfully or intentionally violated any
law, AFP/PMA Rules and Regulations or the honor code of the Cadet Corps Armed
Forces of the Philippines CCAFP and/or consent/allow himself/herself to fail in
character, academic, military and physical standards/requirements of PMA just to be
separated/dismissed from PMA by competent authority for the purpose of evading the
above reimbursement/payment; I/we vehemently understand that I/we will still
reimburse/pay all the expenses incurred by the government in his/her entire period of
training and stay with PMA, after an appropriate investigation made by the competent
authority; and

5. That I/we authorize AFP/PMA to withhold any remaining

balance/financial interest due to my/our son/daughter/ward as a lien for whatever
amount I/we owe to the government (Republic of the Philippines) in case that the
former will be separated/dismissed from PMA as a consequence of the preceding
paragraphs;

6. That I/we undertake to indemnify AFP, PMA or any of its military/civilian

personnel for whatever damage(s) the undersigned or my/our son/daughter/ward may
cause should there be any violation of this Affidavit (Parental/Legal Guardian’s
Consent), any existing law or AFP/PMA Rules and Regulations made by the

undersigned or my/our son/daughter/ward which would be prejudicial to the lawful
interest or prestige of AFP, PMA or any of its military/civilian personnel, any
governmental activity, or would cause administrative embarrassment or unwarranted
injury to any individual regarding the herein Affidavit (Parental/Legal Guardian’s
Consent);

7. Venue. In case of any dispute arising in connection with this document,

the parties hereby agree that the venue for the settlement of the dispute shall fall
exclusively within the jurisdiction of the proper courts in the City of Baguio, Philippines.

8. That I/we executed this Affidavit (Parental/Legal Guardian’s Consent) to

attest to the truth of all the foregoing facts, and for any other lawful intent and purpose
it may serve; and

9. That this Affidavit (Parental/Legal Guardian’s Consent) may be/ was

notarized or accomplished in accordance with Article 114 (Authority to Administer
Oaths) of the Articles of War (Commonwealth Act No. 408) for the purpose of the
administration of military justice and for other purposes of military administration.
