AFFIDAVIT OF CONSENT AND SUPPORT

I, BENJAMIN ELAURZA-LIM, of legal age 36, Filipino, married to ANALYN G.

LIM and a resident of Diamond Homeowners Association, Bgy. Sicsican PPC, Palawan, after
being duly sworn to in accordance with law do hereby depose and say, THAT:

1) I am the father of KRENZ MARIELLE MAE G. LIM who was born on September
24, 2004 in Lazo Birthing Home, Puerto Princesa City;

2) I am giving my consent for my child to travel to Osaka, Japan and accompanied by my

sister MA. AIELEN ELAURZA-LIM.

3) That I am sponsoring her trip and that I am financially capable to support her stay abroad;

4) That I guarantee that she will not violate any existing laws of Osaka, Japan;

5) That I execute this affidavit for whatever legal purpose it may serve in connection with
my child's application for travel clearance from the Department of Social Welfare and
Development
