Prolab Diagnostics is a DOH-accredited clinical laboratory located at the heart of Makati and
Quezon City and has been serving the industry for the last 24 years.

We are pleased to offer our company’s services to provide COVID-19 Rapid Antibody Testing
to your employees/personnel through our laboratory facilities. This is our way of helping assure
that employees as well as companies are safe to resume their works after the quarantine period.

The test procedures shall be handled by competent and experienced health professionals.

Please find below the details of our proposal:

Specifications and Methodology:

 Test to be performed : COVID-19 Rapid Antibody Test (IgM, IgG)

 Sample to be used : Blood through venipuncture
 Use of FDA-approved SARS-COV-2 IgM/IgG assay kit (Colloidal Gold, Lateral
Flow Method). Innovita brand.

Inclusion:
 Health screening (using standard DOH health declaration checklist)
 Collection and testing of samples
 FDA-approved and validated test kits (Innovita brand)
 Competent health professionals
 Protective barriers and disinfecting solutions
 Thermal scanner
 Official laboratory results with interpretation
 Individual medical certificates
 Summary of results
 PPEs
 Maximum of 4 hours on-site service

Requirements:
 List of employees to be tested with corresponding age (to be submitted at least 3
days before the schedule.
 Signed agreement/conforme between Prolab and client or Purchase Order (P.O)
 Ample space where social distancing can be observed (with proper ventilation,
and enough tables and chairs)
 Completely filled-up health declaration and laboratory forms (to be provided by
Prolab)
 Schedule of employees to be tested
 Lead time for scheduling 3-5 days after confirmation
Discounted Price :

P1,200.00 per person (net of tax); waived mobilization fee

Terms of payment:

 Full payment on or before the day of examination (CASH or dated check)
