FORM 11 - FACILITIES AND EQUIPMENT
11.1 Description of Facilities & Equipment: BIDDER shall provide the requested information describing in detail
the following:
 Office layout
 Construction areas
 Marine spreads information, which including:
 List of all Installation vessels (heavy lifting vessel, laying vessel, diving support vessels)
 List of all transportation spreads are utilize for the project: cargo barges & towing tugs, self-
propelled vessels
 Estimated working duration for all marine spreads
 Other facilities and equipment not covered above
BIDDER shall use the table in Attachment F-12-A “CONTRACTOR’s Facilities and Equipment” to
summarize its listing of proposed construction equipment items.

11.2 Marine Vessels: For all proposed marine spreads, Bidder shall provide location information for the 90-day
period immediately following the QUOTATION due date as well as details for procedure to arrange
inspection of the vessels by PTSC M&C/COMPANY Representatives during the 90-day period.
Expected location of BIDDER’s proposed installation barge DLB Kuroshio (K-1) for the 90-day period from
Quotation due date is in Batam, Indonesia or in the Site of other project.
Other support marine vessels shall be outsourced from the market during project execution depending on
their availability. For this reason, BIDDER is unable at this stage to provide the location of other support
vessels.
11.3 Subcontractor’s Information: BIDDER shall also describe in detail the above information for each
subcontractor and any lower tier subcontractors that they intend to use.
The installation barge DLB Kuroshio (K-1) is owned and operated by BIDDER himself. BIDDER shall
perform majority of the offshore installation work using its own in-house resources. As such, there will be
no other major Subcontractor to be engaged for the installation work.
11.4 Detailed Descriptions Attached: The above list is provided as a minimum. BIDDER shall provide as an
attachment to its response a comprehensive list of facilities and equipment required to perform the Work.
Specifications of BIDDER’s proposed installation barge DLB Kuroshio (K-1), DLB Kuroshio II (K-2), ROVSV
(typical), pile hammer and other typical marine vessels are attached herein.

11.5 Inclusion in T&I Subcontract: At time of T&I Subcontract award, PTSC M&C shall select information
submitted by BIDDER relative to Form 11. These submittals, along with any revisions and subsequent
resubmittals, during the evaluation of Technical Quotation shall be included in the T&I Subcontract and
form the basis of T&I Subcontract Exhibit F-12 – T&I SUBCONTRACTOR’s Facilities and Equipment.
