Guidelines on Awards

1. Academic Excellence

FORMAT:

- Rank to two (2) decimals

- To be prepared and submitted by: Adviser 1 / Adviser 2

*Per grade level submission so it must be prepared and signed by all advisers of the same level

- Must be noted by the : Principal

2. Conduct Awards

- Candidates will be taken from the student/s with at least 21 AOs with highest numerical grade (observed values)

- In case of tie, all subject teachers will grade the candidate using the observed values score, then rank

3. Perfect attendance - Per campus / classroom award

- 4th quarter (january to February only)

- no lates, no absences from class and all school activities

- to be awarded on the last day of 4th q exams

4. Leadership Award (grade 6, 10 and other levels that show leadership excellence)

- There will only be 1 awardee per grade level.

STEPS:

1. Determine who would qualify.

HOW: Students submit all documents with summary of achievements in seminars and other extra-curricular activities to the
class adviser. Top 5 highest pointer in extra-curricular points will be the candidates.

* Tentative schedule of deliberation of extracurricular points is 3 rd week of February so all documents must be submitted on
or before the above date.

2. Countercheck on other qualifications (set by DEPEd) with signature from the authorities

a. Have no failing grades in all areas……………………………………………………………………………….. Signatory- Class adviser

b. Have not committed any offense punishable by suspension or higher sanction according to the Department’s service manual

c. Be a class officer or an active member/ officer of any recognized school club,

3. The awards committee will grade the candidate based on the given criteria. ( See template)

Reminders:

A. On Motivational Skills and planning and organizational skills


a. Faculty will be divided by the number of subject teachers.
b. Peers will be represented by 30% of the class population which will be randomly drawn by the class adviser/s from the
class(through fishbowl or lottery method) in front of the candidates. The teacher will then ask the candidates to affix
their signature on the unopened sheet of paper where the peer’s names are written.
B. On contribution to the school and / or community

No more points coming from adviser and peers .

The points will be based extra-curricular curricular points. ( See Handbook)

5. Awards for outstanding Performance in Specific Discipline


- exemplary achievement in athletics, arts, communication arts, mathematics, science, and social sciences.

STEP 1: Candidates will be drawn from among the Top 5 students with most achievement/ winnings (Skills in discipline)on the
specific discipline. IMPORTANT: Only winnings in contests/ competitions will be counted, Not participation .)

1.The club will apply and present achievements with supporting documents and be evaluated by the committee composed of the
school heads and the registrar.
