Program Goal for Your Community:


The goal of this program is to reduce the number of cases of Hepatitis A, Tuberculosis, Typhus and Typhoid
fever among the homeless population of Los Angeles SPA 4 over then next 12 months.

This student gave more detail than necessary, you can list the inputs
Program Inputs: What resources, such as funding, staff, leadership, partnerships, expertise, program
infrastructure, scientific knowledge and evidence-based strategies must occur for you to begin to create
outputs/community Interventions?
The resources that we have in the Los Angeles metropolitan area (SPA 4) include the following:
1. Los Angeles Homeless Services Authority (LAHSA) offers outreach, shelters, safe havens, permanent
housing and other services by coordinating care with partners throughout the city. LAHSA is the lead agency in
the Los Angeles Continuum of Care, which is the regional planning body that coordinates housing and services
for homeless families and individuals in Los Angeles County. LAHSA coordinates and manages over $300
million annually in federal, state, county, and city funds for programs that provide shelter, housing, and services
to people experiencing homelessness (Los Angeles Homeless Services Authority (LAHSA), n.d.).

2. There are several healthcare facilities that will treat homeless patients. Those facilities include clinics as well
as the Los Angels County/USC hospital near downtown LA. A few of the clinics include the Hollywood/Sunset
Free Clinic, Los Angeles Free Clinic, UCLA/Harbor General Hospital, Hubert Humphrey Medical Clinic,
Queens Care, and Watts Healthcare Foundation among several others. I was able to find these listings along
with a brief description of services and population served from a homeless encampment cleanup company
(EcoBear, n.d.).

3. Within the city of Los Angeles, there are several shelters and food kitchens that serve the homeless. The
Union Rescue Mission is the oldest and largest homeless services organization in Los Angeles (Union Rescue
Mission (URM), 2020). The mission provides meals shelter and health and legal counseling clinics in the Skid
Row area of Los Angeles. Hospitality Kitchen and the St. Francis Center are two well know soup kitchens in
the downtown area. However, the St Francis Center also offers other programs such as youth summer camps (St
Francis Center, n.d.).

4. Additionally, a few areas have placed hygiene stations, which include toilets, showers and hand-washing
basins with soap, (Homeless Healthcare Los Angeles, n.d.). Since these illnesses are spread easily among the
homeless, having someone talk to homeless people utilizing the resources listed such as shelters and food
kitchens about how to prevent the spread of the diseases would be a cost effective way to get the information to
a large number of high risk people. However, knowing that hand-washing is important is only part of the
problem.

5. Many homeless have limited ability to maintain proper hygiene and so providing hand sanitizer and
facemasks to homeless people through these service centers and working with the city to place more hygiene
stations would also be of great benefit. Also, there is an effective vaccine for Hepatitis A that could be given to
high-risk populations including the homeless if they wanted it. Finally, a poster with information about these
diseases including symptoms, prevention and where to get treatment could be produced and placed in shelters,
food kitchens, public restrooms and on bus benches. Within the community there exist partnerships and special
funds allocated by the city of Los Angeles and the state of California that are for the benefit of the homeless
population. A starting off point for the small amount of funds needed for the creation of posters and providing
masks and hand sanitizer would be to reach out to area hospitals, and LAHSA for donations and or grants/funds.
Objective 3 (Long -Term)
By December 2020, the number of homeless people in Los Angeles presenting to clinics and hospitals with
infections from Hepatitis A, tuberculosis, Typhus or typhoid fever will drop by 50%.

In order to give the program time to become fully engaged, we would not
start the long-term evaluation until mid December 2020 and then monitor
the reported cases annually thereafter.
