DISCIPLINARY ACTION PROCEDURE 7.4 Enforcement of HSE Norms & Procedure Purpose: To impose Enforcement action in case of HSE violations at work site, ‘Scope: All employees at the AFCONS projects sites Enforcement action is summarized below: 1st Warning: Verbal intimation of the violation or company concern. The employee's immediate supervisor wil be notified. Record of the verbal intimation will be placed in the ‘employee's personnel record .Counseling to be conducted by the activity in charge & HSE Officer. This shall be placed in record 2nd Warning: Written Employee intimation of the violation. Written notice will be issued to the employee, with a copy to employee's supervisor, and to the employee's personnel file. Company policy provides that this intimation may be removed from an employee's personnel file after period of (3) months, if no similar warnings are issued to the employee within that time period. Counseling session shall be conducted by the Admin in charge / Site In charge / HSE in Charge. A waiting period of atleast three months shall be given to observe improvement 3rd Warning: Employee / workman to be removed from the site of activity for the day. Record of the incident will be placed in the employee's personnel fle. The Site in Charge & HSE In charge shall conduct counseling session. Mandatory Warning: In case of Fatality / LTI / Dangerous occurrence a letter will be automatically issued to the concemed site in charge & the HSE officer .This will be withdrawn if there is no further incident for Six Months from that date. Employees receiving such warnings will be excluded from the safety incentives schemes for six months.
