ANNEXURE 7- DECLARATION BY THE SUBCONTRACTOR FOR HSE COMPLIANCE

Project: “Improvement of Dharkhar to SaraiI (Ch. 54+000 m to 81+054 m) of Cumilla (Moinamoti)-

Brahmanbaria(Sarail) Highway(N-102) as 4-Lane National Highway including all structures,
earthwork, foundation etc”.(WP-02)

Subject: Subcontract for Piling work as per the design and drawings for “Improvement of Dharkhar
to SaraiI (Ch. 54+000 m to 81+054 m) of Cumilla (Moinamoti)-Brahmanbaria(Sarail)
Highway(N-102) as 4-Lane National Highway including all structures, earthwork,
foundation etc”.(WP-02)

I/We hereby declare that we have studied the RFQ documents and warrant that we shall
fully comply with and conform to the Technical requirements of the RFQ Document for the
subject work without any deviations.

I / We further declare and certify that,

1. We agree with the code of conduct specified in Afcons HSE Enforcement and
Encouragement Procedure appearing herein as Annexure 8
2. We shall follow the Site HSE policy for doing the work.
3. Resources:
a. We shall employ skilled and competent labour for the job which shall be able to
follow the HSE / safety rules laid down by the Site
b. The Equipment employed by us for the Subcontract work shall be in good safe
condition to operate with sufficient factor of safety.
c. We shall depute a dedicated HSE officer at site to ensure compliance of HSE
requirements.
4. *OHS / Accident Performance: We confirm that there has been no fatal or major
accident during the last 3 projects executed by us.

(*Note: In the event of occurrence of any such incident during the past three Projects
executed by Subcontractor for any Company, the Subcontractor has to provide details of the
same. Incorrect declaration shall be considered as a material breach and the Subcontractor
shall be responsible for all liabilities / consequences resulting from the breach)
