AFFIDAVIT OF EXPLANATION

I, SANDEE C. HILARIO, of legal age, Filipino, married, presently residing

at # 308 Mabini Street, Poblacion Street, Science City of Muñoz, Nueva Ecija,
under oath, state:

1. I am the owner of AMYSAN TRADING located at # 308

Mabini Street, Poblacion West, Science City of Muñoz,
Nueva Ecija.

2. I am applying for renewal of Business Permit before the

Licensing Office, Municipal Hall at Science City of Muñoz,
Nueva Ecija and one of the requirements was to submit an
affidavit stating thr gross income of my business.

3. Unfortunately the competitors was increasing which made my

profits decreased from P 1,440,000 on year 2018 to P1,0
80,000 on year 2019.

4. That, I am executing this affidavit to attest to the truthfulness

of the foregoing facts and to inform the Licensing Office,
Municipal Hall at Science City of Muñoz, Nueva Ecija and
for whatever legal purpose it may serve.
