AFFIDAVIT

SPOUSES GEOVANNI R. SOLIS and ATHENA CESARIA ANDREA P.

SOLIS, both of legal age, Filipinos, presently residing at 144 Roxas, Talugtug,
Nueva Ecija, after having been sworn to in accordance with law, hereby depose
and state, THAT:

1. That WE are the lawful owner of residential house located at

Roxas, Talugtug, Nueva Ecija;

2. That ERNESTO SOLIS, the former owner of the said residential

house was the registered owner of Account No. 0601120460
concerning the water supply in the above stated residential house;

3. That WE hereby request TALUGTUG WATER DISTRICT to

transfer in our name the registration of said meter and assume all
the rights, responsibility and obligations covering said
TALUGTUG WATER DISTRICT account;

4. That WE will behold ourself criminally and civilly responsible for

any liability that may arise in connection with the transfer of the
above cited under Account No. 0601120460 in my name and
further free TALUGTUG WATER DISTRICT from liability
relative thereto;

5. WE have executed this affidavit to attest to the truthfulness of the

foregoing facts.
