JOINT AFFIDAVIT

WE, MARIFA N. BALADAD, 52 years old and HELEN N. DADAG, 57

years old, Filipinos and residents of Barangay Mapangpang, Lupao, Nueva Ecija
and after having been sworn to in accordance with law hereby depose and state,
THAT:

1. We personally know SPOUSES ORLANDO NARDO NITURA, SR and

GLORIA G. NITURA, who got married on March 27, 1965 at San Mateo,
Isabela.

2. We likewise know that ORLANDO NARDO NITURA, SR. died on October

7, 2019 at Brgy. Mapangpang, Lupao, Nueva Ecija. Attached herein is the
copy of Certificate of Death and marked as Annex “A” and made an integral
part hereof.

3. These facts, are of our own true and personal knowledge, for we are the
relatives of SPOUSES ORLANDO NARDO NITURA, SR and GLORIA
G. NITURA.

4. We executed this affidavit without promise of reward and that we will not
be benefited for its execution.

5. We have executed this affidavit to attest to the truthfulness of the foregoing

facts and for whatever legal purpose it may serve.

IN WITNESS WHEREOF, we have hereunto set our hands this October 18,
2019 at Science City of Muñoz, Nueva Ecija.
