SWORN AFFIDAVIT with WAIVER OF RIGHTS

I, GLORIA A. BUENO, of legal age, Filipino, widow, presently

residing at Barangay San Antonio, Science City of Muñoz, Nueva Ecija,
after having been sworn to in accordance with law, hereby depose and state,
THAT:

1. I applied for a free patent involving lot no 140-A of Csd-03-

018402.

2. I am no longer interested in pursuing the said application, and I

entrust the said application to Manolito Bueno.

3. Furthermore, I do hereby waive, relinguish and renounce all rights,

interest and participation in favor of Manolito Bueno over the
above-mentioned parcel of land.

4. I have executed this affidavit to attest to the truthfulness of the

foregoing facts, to inform the authorities concern about the facts,
and for whatever legal purpose it may serve.
