AFFIDAVITOFCONSENT

I, MARILES M. UYCHOCO, of legal age, single, Filipino, with address at

San Benito Sur, Aringay, La Union, after having been sworn in accordance with
the law, herby depose and states that:

I am the administrator of a parcel of land located at Cacapian, San Juan, La

Union covered under TD No. 2009-15-0012-00916 consisting of 39,130 square
meters registered in the name of CESAR D. UYCHOCO on which lot I allowed
HELWIN CABADING PALAROAN to apply for electric connection (with
meter) with LUELCO (Bacnotan);

That HELWIN CABADING PALAROAN is applying for the foregoing to

be installed at his residential house built on the same above described property;

As a consequence, I am giving my full consent over his application and

executing this Affidavit for the foregoing purpose and for all legal intents and
purposes it may serve.
