 Tank explode cause 3 workers were killed 7 workers were injured
 PCA’s not apply it’s process safety management system
 Process generates vapors that contain water, turpentine & various sulfur compound
 Maintenance is Repair water piping above foul condensate tank
 Foul liquid remained in tank 10 ft cause work’s not directly to the tank and company
assumed that tank contained mostly water turpentine’s be skimmed off and sent to
recovery system but this month valve designed to direct skimmed was close
 Flammable turpentine in foul condensate tank
 Tank is cool during shut dawn by low pressure in tank so release valve on the tanks roof add
air to tank to avoid vacuum O2 is enough to combustion to create an explosive

Initiation

Foul condensate remains in tank during planned maintenance shut down

1) Worker detect a flammable gas around tank and they continue to work because work isn’t
directly to the tank work repair water pipe above the tank
2) Turpentine ‘d be skimmed off an sent to recovery system but during shut down valve was
close
3) During shut down tank is cool, so low pressure condition is occurred in tank so release valve
on the tanks roof add air to tank for avoid vacuum in take cause an oxygen level in tank is
enough to combustion and explode
4) Spark or molten slay produced from hot work landed on or near the foul condensate tank
heating up and igniting its content

Termination

1) Always drain /purge before shut down

2) Use tank that can endure low pressure condition
3) PHA (process Hazard analysis) must be applied to factory
4) All industrial equipment must have moderator or section that undertake
