AFFIDAVIT

I, CYRIL W. RANILE, Filipino, of legal age, single and a

resident of 96 Vamenta Boulevard, Barangay Carmen, Cagayan de
Oro City, Philippines, after having been duly sworn to an oath in
accordance with law hereby depose and state:

1. That I am an incumbent member of the Sangguniang Barangay

of Barangay Carmen, Cagayan de Oro City from 2013 up to
present as evidenced by the Certification dated September 16,
2019 issued by the Department of the Interior and Local
Government, Region X – Northern Mindanao and signed by the
Chief, Milagros A. Felisilda.

2. That during the term, I was not employed in any other

government office.

3. That also, I did not receive any form of salary/compensation,

except honorarium as an elected member of the Sangguniang
Barangay of Barangay Carmen, Cagayan de Oro City.

4. That finally, I execute this affidavit to establish the truth to the

foregoing statements and for whatever legal purpose it may
serve.

IN WITNESS WHEREOF, I have hereunto affixed my signature

on 19th day of September 2019 in the City of Cagayan de Oro,
