AFFIDAVIT

I, ANSARI ANDAMUN SALIC, of legal age, married, Filipino,

and a resident of Barra Macabalan, Cagayan de Oro City, after having
been duly sworn to in accordance with law, do hereby depose and
say:

1. That I am the true and actual owner of a business

establishment, Junie Lim Reloading Station & General
Merchandise situated at 4th Floor, Gaisano Mall, Barangay
26, Cagayan de Oro City.
2. That per Business Name Registration Certificate No.
04118307, the registered owner of the above-mentioned
business is Junie Tion Lim, who is a resident of Block 5, Lot
22, Purok 2, Citi Homes Subdivision, Malang, Opol, Misamis
Oriental.
3. That in truth and in fact, I am the true and actual owner of
the said business establishment although it is registered
under the name of Junie Tion Lim.
4. That the registration under his name was necessitated by the
documentary requirements of the mall, where it is situated.
5. That all my business transactions have been conducted in
the establishment for almost three (3) years now.
6. That I execute this affidavit as to the truth of the foregoing
facts and for whatever legal purpose/s this may serve.
