AFFIDAVIT

I, LALAINE SANTOS MAGAMPON, of legal age,Filipino, married, and a

resident of Purok 6, Barangay Palma Dos, Palma Alaminos, Laguna, after having been
duly sworn in accordance with the law, do hereby depose and state:
1. That, I am putting up my own water refilling station in Palma Dos, Alaminos
Laguna;

2. In doing so, That, I have duly assigned my representative to facilitate with the
application and processes involved in securing the needed permits and licenses;

3. On that same note, That, I have secured a Business Registration Permit with
Department of Trade and Industry using my maiden name, Lalaine Flores Santos
with registration date of February 27, 2018 and Certificate No.05240635;

Attached hereto is a copy of the said DTI permit, marked as ANNEX “A”;

4. That, I have every intention and purpose to cancel the above-

mentioned DTI Permit and apply for a new permit bearing my married name,
Lalaine Santos Magampon;

5. That, I was married to Noel Magampon last, June 17, 2017, in Alaminos, Laguna
at Our Lady of the Pilar Parish;

Attached hereto is a copy of the marriage certificate issued by the Philippine

Statistics Authority, marked as ANNEX “B”;

6. That, I have executed this affidavit to declare all stated herein as well as attest to
the truth of the foregoing facts;
