PETITION
COMES NOW, Petitioner Hera R. Ortiz, by the undersigned counsel,
and unto this Honorable Court, most respectfully avers:

1. Petitioner is the maternal grandmother of minors Pia Ortiz Ignacio

and Mia Ortiz Ignacio, Filipino, of legal age, married to Juan Borlado
Ortiz, has the legal capacity to sue and be sued and a resident of
Naga City, Camarines Sur, where she may be served with summons
and other processes of this Honorable Court;

2. Petitioner is also the mother of Marla Ortiz Ignacio, the mother of

minors Pia and Mia Ignacio, who died on 10 March 2018 at St. Luke’s
Hospital Bonifacio Global City, Philippines. Photo stat copy of her
Death Certificate is hereto attached as Annex “A” and made an
integral part of this Petition;

3. Minor Pia Ortiz Ignacio is likewise a Filipino, born on 18 April 2007,

at Naga City, Camarines Sur, and presently residing with the
Petitioner at Naga City, Camarines Sur;

4. Minor Mia Ortiz Ignacio is likewise a Filipino, born on 23 September

2008, at Sangley Point, Cavite City, Cavite, and presently residing
with the petitioner at Naga City, Camarines Sur;

6. That due to the minority of the said MINORS, it is necessary

and convenient that a guardian over her person and property be
appointed;
7. That, as above stated, Petitioner is the person having the said
MINOR in his care, and that she possesses all qualifications of a
person to who letters of guardianship should issue;

8. That Petitioner has furnished a bond amounting to One

Hundred Thousand Pesos (P100,000.00) or Ten Percent (10%) of the
value of the property owned by the MINOR.

PRAYER
WHEREFORE, it is respectfully prayed that, after due notice and
hearing, petitioner be appointed guardian over the estate of the
MINOR.
