My client, MARILOU CERVANTES sough the legal assistance of my office regarding her
outstanding obligation with your office in the amount of THREE HUNDRED FORTY-TWO
THOUSAND TWO HUNDRED EIGHTY-NINE PESOS AND 80/100 (PHP 342,289.80), inclusive of
the interest, penalties and other charges.

In this regard, may we respectfully ask for a copy of the STATEMENT OF ACCOUNT of the
aforementioned borrower, reflecting therein, the principal loan, interest and other charges
already due and demandable.

As provided under Section 4 of Republic Act 3765 or otherwise known as “Truth in Lending Act”,

Should you fail to settle your account with our client within the said period, then we shall be
compelled, much to our regret, to file the appropriate cases against you with all the rigors of
court litigation, the shame and inconvenience of public trial, including the attendant demands
for damages, attorney’s fees and litigation expenses. In addition, we might avail of the remedy
of attachment to garnish your bank deposits and attach your properties. To avert a full-blown
trial, you may come to the office of my client to come up with possible remedies which would
be convenient to you and acceptable to our client.
