WAIVER OF LIABILITY

I, ____________________________, the Borrower of a certain housing unit,

indentified as Lot ____, Blk ____ of Anika Homes, Visayan Village, Tagum
City, freely and voluntarily execute this waiver of liability in favor of the
Developer of Anika Homes, Purok Pioneer, Visayan Village, Tagum City,
Davao del Norte, under the following terms and conditions:

1. I hereby release, waive, and forever discharge the Developer of

Anika Homes, Purok Pioneer, Visayan Village, Tagum City, Davao
del Norte, and its partnering organizations from any and all
liability, claims and demands of damages of whatever kind either in
law or in equity, which may arise after my take-out and receipt of
keys over my completed housing unit, indentified as Lot ____, Blk
____ of Anika Homes, Visayan Village, Tagum City;

2. I will fully assume all the responsibilities, obligations, and risk of

property damage caused by owner’s negligence or by accident or by
any fortuitous event;

3. I fully understood that my said housing unit is covered by

insurance scheme/coverage, under the auspices of PAG-ibig Fund;
