Preferred Supplies Vendor Registration Application Form

(To be Filled Up on Manufacturer’s/ Suppliers as “Vendor Letter Head)

• Registered office/Correspondence office Address,Email Id & Contact Details
• GST/CIN/IEC/VAT/Trade License/ Sales Tax/ TDS Registration details and
copy as applicable
• Product list & product description along with Brand name in which its
supplied
• Details of sizes in which packaging will be done & supplied
• FSAAI/Trademark/APEDA/Organic etc other mandatory registration details
and copy as applicable
• Provide copies of FSSAI approved lab test reports, Nutrition Lab report and
other such reports from government approved certified agencies for each
product supplied as applicable
• Details of the Manufacturing Unit along with Registration & Licenses Copy
• Supplies will be made available for Pan INDIA & International Orders
• MRP to be Printed on each Product Supplied
• EX-Factory Price to be quoted to GMFH on each Product Supplied
• Terms of Payments:
50% Advance , remaining 45% on Dispatch and remaining 5% post
inspection at delivery source
• Delivery Terms: Once 50% advance is placed in 7 days dispatch
• Bank Account Details: Type of account, Account Number,I FSC code,
Branch Address, Banker email id & contact details, cancelled cheque
• Permanent Account Number details and copy
• Last 3 Years Audited Financials & ITR Returns Copy
• Authorised/Power of Attorney Copy from the Manufacturer’s/ Suppliers as
“Vendor” to deal with GMFH
• Corporate profile with details of ownership /directors /proprietors as
applicable
• All the payments to Vendors will be directly made by National Cooperative
Consumers Federation of India Limited (NCCF) for the orders placed by
them directly as our Joint Venture partners for NCCF related Supply orders.
• Further,all the payments to Vendors will be directly made by GMFH for the
orders placed by GMFH directly for supplies other than NCCF placed direct
orders.

 Declaration by the Manufacturer’s/ Suppliers:

*All Quantity, Quality , Adulteration if any ,Packaging & Timely delivery of

supplies is the sole responsibility of the Vendor and ensure necessary
warranty/guarantee for the same.
*No upward price revision to be taken without prior notice & specific consent
of GMFH for atlas 1st 4 moths from the date of registering as preferred
vendor with GMFH
*In case Post Inspection there are issues regarding the same then
immediately the supplies have to be replaced by the Vendor.
• The Vendor has to comply with Package Commodities Act .1977 and the
PFA Rules/ISI Specifications.
*All timely legal compliances, statutory approvals and renewals of all
applicable certifications & licenses is the sole responsibility of the Vendor to
adhere and provide a copy of it to the compliance team of GMFH.
*Proper receipts, invoices , challan copies , all related documentation to be
timely provided by the Vendor to the compliance team of GMFH so that no
liability or dues of any sort is charged on GMFH.
*Proper Audit , Books of Accounts & Tax Compliances to be timely done &
adhered to and shared with compliance team of GMFH so that no liability or
dues of any sort is charged on GMFH.
*All customer complaints have to be timely & completely to be addressed to.
* As Vendor we have never been black-listed by any government Agency.
• The operating corporate entity of GMFH entering into all such agreements
will be:

similar products in the market. We shall be solely responsible for any
complaint regarding the quality/quantity/packaging/price deviation from
agreed and rates of items supplied by us and undertake to make good
losses, if any to GMFH out of our dealing with GMFH.
• That in case any disputes arising due to offer/ contract/ supply should be
settle by arbitration with Delhi as jurisdiction.
That the information and documentation given with the application form to
GMFH for registration as preferred supplies Vendor are true and correct.In
case any discrepancies are found, we should be solely responsible for it.
