    Documents required for Medical Visa/Medical Attendant Visa

    1) Fully Filled in Regular Visa Form (Attached herewith)


    2) Fully filled in Undertaking (Attached herewith)
    3) NID/Birth certificate,
    4) Latest Utility Bill copy
    5) Bank statement/Dollar endorsement (Endorsement should not be older than 1 (one) month at the
    time of submission) /Active international credit card /Travel card
    6) Medical Invitation letter from India with specific date
    7) All Medical original documents
    8) Profession proof
    9) Last passport copy and all old passports
    10) 10) Clearance from Ministry of Foreign Affairs, Bangladesh (In case of organ transplant
    patients)
    11) One recent (not less than 3 months old) 2x2 (350x350 pixels) colour photograph
    depicting full face with white background.
