Sir/Madam:

This has reference to the Notice to Correct Violation issued by this office on
__________________for compliance of your establishment/building located at the above
cited address. Despite the length of time that has elapsed and the re-inspection report under

complied.

In this regard, you are hereby imposed an Administrative Fine of
