ABATEMENT ORDER
Sir/Madam:

This has reference to the Notice to Correct Violation issued by this office on
__________________for compliance of your building, facility or structure located at the above
cited address. Despite the length of time that has elapsed and the re-inspection report under
Inspection Order No. _____________ dated _________ fire safety requirements remain not
complied.

In this regard, you are hereby imposed an Administrative Fine of (Php)__________

pursuant to Rule 13 of the RIRR of RA 9514. Hence, you are given three (3) days to pay the fine

Further, you are again directed to comply with specific fire safety requirements in
accordance with the provisions of RA 9514. The following violations/deficiencies are hereby

Failure on your part to pay the administrative fine/to correct the deficiencies within the
period specify in the Abatement Order shall constitute a ground for the issuance of Closure
Order for your building or the portion thereof and payment of administrative fine does not
absolve in compliance of the fire and life safety requirements.
