CLOSURE ORDER

This CLOSURE ORDER is hereby issued by the undersigned against your

building, facility or structure, after the same is found to have consistently FAILED TO
COMPLY with the fire safety measures/non-payment of administrative fine pursuant to
Section 9 of RA 9514 otherwise known as the “Fire Code of the Philippines of 2008” and
Rule 13 of its Revised Implementing Rules and Regulations (RIRR).

The closure of your building, facility or structure shall be effected immediately

upon receipt hereof, either by yourself or thru a representative, in order to avert any
unwanted danger that your building, facility or structure may or might cause to the
general public.

This Order shall remain valid and effective unless ordered revoked or annulled by
a competent authority.
