I. ACTION REQUESTED

1. For the Regional Director, BFP Caraga, to approved the request for the purchase of
office machinery such as printer from 20% of LGU share.

II. BACKGROUND JUSTIFICATION

2. That, this station has only one (1) unit of printer which has been a communal used of
all personnel.

3. That, this situation may cause and affect inconveniences to perform the duties and
responsibilities both in operation and admin works.

4. That, the approval of this request can contribute strength and provide what is
necessary to accomplished for the betterment of Sta. Monica Fire Station.

III. RECOMMENDATION

1. For the approval of the paragraph 1.
