Tanker with Plate No. with Plate no. CAB 5780 of Morong Fire Station, Rizal BFP
Region 4A was made for the following reasons:

1. The damages or defects of the above-mentioned vehicle/firetruck were

caused by ordinary wear and tear and not through the fault, negligence, or
carelessness of the accountable/responsible officer or any other employee;

2. The need for the vehicle is exceptionally urgent and absolutely indispensable
in that delay in its repair would cause detriment to public service and this is to
prevent immediate damage to or loss of life and property;

3. That the price obtained for labor and materials is the lowest obtainable in the
market at the time of the repair;

4. That the repair / work was done in accordance with the job specifications /
Approved Work Order.

This certification is being made in accordance with the requirements of Sec 6,

Executive Order Nr 262 Series of 2000.
