How to register to Servizio Sanitario Nazionale

Once you have your CODICE FISCALE please follow this procedure to register to the public health
system. You will be registered for one year, starting the day you send your registration request.

1. Payment: Go at any postal office Poste Italiane and pay the annual contribution of €149.77
for the voluntary registration using a “bollettino postale” that you must fill up with the
following information:
a. Sul CC n. 289504
b. Di Euro 149.77
c. Intestato a Regione Toscana
d. Causale: CONTRIBUTO ISCRIZIONE VOLONTARIA AL SERVIZIO SANITARIO
REGIONALE ANNO 2020
e. In the bottom part insert your information

2. Register: Be careful not to lose the receipt of your payment. As soon as you have paid your
registration:
a. Send an email to iscrizioni.volontarie@asf.toscana.it with the following
documents attached:
i. ID or passport
ii. Your Codice Fiscale
iii. The receipt of the payment
iv. Letter of engagement signed by you and the President of Mascarade Opera
Studio demonstrating your enrolment in the school
v. Scheda Statistica (you can find this attached in the email) with all
information required. You have to tick “Prima iscrizione” and “Tipo di
contributo” FORFETTARIO (indicate the amount and date of payment made)
Please note:
In the Scheda Statistica you will be asked to indicate your general practitioner. Our suggestion is that you
choose between one of the following doctors who work in the ambulatory in Via al Prato next to the
pharmacy (other side of the street from the Studio) and speak decent English.
