 AUTHORIZATION FORM FOR INVOICE SENDING BY E-MAIL

Dear Customer,

the actual law (Resolution no.107 dated 4/07/2001) about electronic invoices and computer documents allows the
companies to send fiscal documents via e-mail REPLACING the traditional postal service.
This system allows to receive the documents shortly and avoiding losses or delays.
The receiver will have to print the computer document on paper, replacing the original copy of the invoice itself. The
invoice that we will send by e-mail will have to be printed and stored by the receiver.

In order to take advantage of the service, we need your explicit consent.

Any revocation or amendment of this authorization may be made by either party with written notice.
