GUIDELINES: The Employee Performance Appraisal Report is divided into 4 Parts:
Part A: Performance Evaluation on KRAs (Page 2) This section would be utilized for tracking Individual Performance on the agreed Key Result Areas and Key Performance Indicators of his/her KRA/KPI Sheet. The following aspects will be covered under the same: 1) Performance Planning: This process has to be completed in the initial 3 weeks of the Performance Appraisal Year. The Allocation of KRA Weights & Targets has to be completed as a Part of this module. This should be completed after discussion between Appraisee & Reporting Officer. The same should be duly reviewed & approved by Counter Signing Officer (CSO). 2) Mid-Year Review: Individual’s achievement against the targets set for his KPIs would be reviewed during the Mid Year Performance Review. It is essential to ascertain that the targets set are relevant. It is also essential to have a discussion with their Reporting Officer for resource allocation to enable achievement of the Annual Targets. Moderation in targets is allowed under specific circumstances on approval by CSO. 3) Final Performance Evaluation: This process shall be conducted within the 1st / 2nd month after closure of Performance Year. Individuals would be assessed on their performance on each KRA. The Reporting Officer would be rating the appraisee based on actual achievements using the 5 point rating scale for evaluation. Part B: Performance Evaluation of Qualitative Parameters/Competencies Section (Page 3) This section provides for assessment of individuals on skills required to perform his/her work efficiently. The assessment shall be conducted within the 1st / 2nd month after closure of Performance Year. The Appraisee can take this opportunity to write comments on each of the Qualitative Parameters. The Reporting Officer would be rating the individual, by using the 5 point rating scale for evaluation of Qualitative Parameters/Competencies. Part C: Performance feedback (Page 4) The Appraisee will write his/her self appraisal in the space provided. The Reporting Officer shall provide overall Performance Feedback to the Appraisee. This will need to be documented. CSO & Accepting Authority will provide Feedback on Performance Part D: Final Score (Page 4) The final score shall be calculated based on the scores arrived at in: Part A: Performance Evaluation on KRAs/KPIs (80%) & Part B: Performance Evaluation on Qualitative Parameters/ Competencies Section (20%) The final score is arrived at using a weighted average. The weightages for Part A and Part B are indicated above. The Accepting Authority will close the process with his/her comments.
