SUBJECT: Grave Warning Letter

TO: Staff Marcelo Hospital Renal Center

This letter is to serve as a Grave Warning Letter to your center due to the resistance to follow
direct orders from the management. Nephrolife Dialysis Center is having a crisis and in need of
additional staff to reinforce their manpower, and Mr Franklin Ruiz was chosen to response. It is
written in signed contract of all staff that they can be transferred to another center if needed.
The center doesn’t have the right to hold transfer of any of its staff.

As a sanction, the medicines incentives of the center for the 3rd quarter will be on hold.
