    SUBJECT: Grave Warning Letter

    TO: Mr. Franklin Ruiz


    Staff Nurse – Marcelo Hospital Renal Center

    This letter is to serve the Grave Warning Letter due to various reasons stated below:
    1. Resistance to follow direct orders from your superiors
    a. You are instructed to transfer to Nephrolife dialysis Center to help with the
    shortage of staff due to a crisis.
    b. It is written in your signed contract that you can be transferred to another center
    if needed.
    2. Unauthorized use of medicine income
    a. Being the medicine coordinator, you are tasked to monitor the medicine ‘s sales
    of the center, and you are not authorized to use the money for your personal
    gain. You should have asked for permission before doing so.

    Please sign this letter indicating that you have read and understand the content of this letter.
