On behalf of my client MR. JAIME R. VERDEJO, I would like to request for a

Certification from your good office that the parcels of land designated as Lot No. 13389,
with an area of 2.2825 hectares, covered by Tax Declaration No. 00047-00138 and Lot
No. 13393, with an area of 0.4960 hectares, covered by Tax Declaration No. 00047-
00275, both registered under the name of Federico Ustaris, are covered by Presidential
Decree No. 27, s.1972. To support this request, we are attaching copies of the following
documents for your ready reference.

1. Tax Declaration No. 00047-00138 for Lot No. 13389, registered under Federico
Ustaris;
2. Tax Declaration No. 00047-00275 for Lot No. 13393, registered under Federico
Ustaris;
3. Order for T.A. Case No. D-885 with the caption “IN RE: REALLOCATION OF
FARMLOT/HOMELOT WITHIN THE ESTATE OF AURELIO USTARIS
SITUATED AT SOGOD, CALABANGA, CAM. SUR” issued on September 13,
1994 by DAR Region V and signed by Romeo S. Perez, Jr. OIC-Regional
Director , which designated JAIME VERDEJO as the sole-owner/cultivator of the
lands stated therein; and
4. Consolidation-Subdivision Plan- Swo-05-001498 (OLT)
