MOTION FOR THE ISSUANCE OF SUBPOENA DUCES TECUM AND AD

TESTIFICANDUM

With all due respect to the Honorable Court.

Plaintiff, thru undersigned counsel, unto this Honorable Court, respectfully

alleges that:

1. In order for the plaintiff that he is one of the heirs of the late O, he needs
the testimony and documents from the Philippines Statistics Authority
and/or the Local Civil Registrar of _____;

2. Hence, the plaintiff prays that a subpoena duces tecum and ad

testificandum be issued on the Regional Director-Region 1, Philippine
Statistics Authority and /or Local Civil Registrar of Luna, La Union to bring
and testify on the following documents:

I. Birth and Death Certificates of the following siblings:

a. Y
b. H
c. T
d. 5
e. D.

II. Birth and Death Certificate of the following siblings:

a. R
b. F
c. D

III. Marriage Certificate of R AND O

IV. Birth Certificate of E

3. These documents should be brought and testified to on October 1, 2020

at 9 A.M. ;

 4. This motion is not intended to delay the proceedings but solely for the
above-stated purposes.

PRAYER

Wherefore, it is being respectfully prayed that the Court issues a subpoena

duces tecum and ad testificandum to the representative of the Philippine
Statistics Authority-Regional Office No. 1 and /or Local Civil Registrar of Luna, La
Union, to bring with them the documents above-enumerated and to testify
thereon.

Other just and equitable reliefs are likewise prayed for.
