2.12 Emergency Response Systems

What

This procedure is developed by Dalma Energy Algeria and defines as the Dalma Energy
operations will ensure that emergency response activities are carried out with the
following priorities.
 Safeguard Lives
 Protect the Environment
 Protect Assets
 Maintain Dalma Reputation

Personnel involved in dealing with the emergency will follow these priorities when making
decisions and formulating an action plan.

Emergency Response Systems on Dalma Rigs will consist of the following major areas:

 Provision of Equipment
 Emergency Procedures and
 Emergency Response Drills.

Who

Drilling Superintendent

Respective DS is responsible for Well control Equipment to comply with respective


Drilling contract. He will also confirm that if contract demands, Ambulance shall be in
place.

Chief Mechanic

Chief Mechanic is responsible to maintain Assembly points (primary & secondary area)
& BA sets storage facility.

Medic

Medic is responsible to maintain Eye wash stations at mud mixing area, shaker area, rig
floor & workshop. He will ensure that standard first aid boxes at rig (TP Office, rig floor,
workshop) & camp mess rooms are in place & maintained.

Tool pusher

Tool pusher will ensure that one normal stretcher & one with crane lifting facility is in
place. He is also responsible to maintain search routes maps at both assembly points &
wind socks at Dog house, mud tank, workshop office roof & at assembly point.
He will be responsible to maintain respiratory protective equipment, alarms & ensures
that Safety signs & instructions, mainly at rig & camp direction signs are suitably placed
to guide ambulance or fire tenders. Rig alarm sign, electric shock treatment sign, well
shut in instructions inside the dog house, H2S response instruction in dog house, fire
response instruction in dog house, layout maps indicating safety equipment on the rig &
camp & TP office are displayed.

Tool pusher make sure that emergency information Notice board at Rig (TP’s office) &
camp (Senior Mess) displaying all up-to-date contact numbers of nearby hospitals /
clinics, client emergency support stations if exist, nearest police stations, fire stations, fire
team list & first aiders list.

As per schedule TP will conduct emergency exercise & all drills will be recorded. TP will
be responsible to implement emergency response system at rig site & actively Involved
in emergency response plan.

Warehouse Supervisor - Algeria Base

At the Algeria base ware house, warehouse manager is Responsible for implementation
of emergency response system.

HR Manager - Algeria Base

At base office Manager HR/Administration is responsible for implementation of


Emergency response system.

How and When

In order to achieve the above, the Company prepared the following arrangements
through its HSE Strategy.

1. Provision of Equipment

The following equipment will be made available on each rig for suitable response to any
emergency situation:

- Assembly Points (Primary and Secondary) with BA Sets storage facility.

Fire response instruction in Dog House & TP Office)
- Layout maps indicating Safety equipment on the rig & camp.
- Well control Equipment
- Emergency information Notice board at rig (TP’s office) & camp (Senior Mess)
displaying all up-to-date contact numbers of nearby hospitals/clinics, client
emergency support stations, nearest police stations, Fire Team list, First Aiders
List & Safety Equipment Layout.

2. Emergency Response Procedures

Emergency response procedure is prepared for the following emergency situations on


the rig location.

o Medical Emergency & Evacuation (Medevac) – Serious injury or illness of


personnel.
o H2S- H2S gas release on location
o Fire- Major fire on the rig or at the camp
o Blow out- For uncontrolled Well kick
o Missing Person- Search for missing person
o Rig Failure- Rig structure collapses, foundation collapse or complete power
failure when the Well is open in hydrocarbon bearing zone
o Environmental Spill- Uncontrolled release of oil or chemicals on the rig
location that could endanger people’s lives, cause severe damage environment or
cause damage to assets.
o Floods or Natural Disaster- Flood, cyclone or earthquake related situation
o Vehicle hijacking

3. Emergency Response Drills

The following Emergency Response Drills are mandatory on all Dalma Rigs in Algeria
Operation.

o H2S Drill- (Monthly)


o Fire Drill- Rig (Fortnightly)
o Fire Drill- Camp (Monthly)
o BOP Drill- During Tripping, during drilling(Weekly)
o Stretcher Drill- (Monthly)

4. Emergency Response Equipment – Base office & Warehouse

The following Emergency Response Equipment is available at base office & in


Warehouse:

o Assembly Point
o 1 Stretcher (Folding type)
o Fire Alarm
o Emergency information Notice Board containing all up to date contact numbers of
hospitals, police station and fire stations.

5. Emergency Response Procedures- Base office * Warehouse

The Emergency Response Procedure is prepared for Algeria operation for the following
emergency situation applicable to base office & Warehouse facility:

o Medical Emergency & Evacuation – Serious injury or illness of personnel

The Emergency response procedures is address the following elements as a Minimum:

Algeria operation will maintain an up-to-date emergency contact numbers list including
Local authority & hospitals. The local emergency number will be displayed prominently
on all notice boards.

6. Guidelines on how to behave in hijacking or kidnapping situation:

The chance of becoming the subject of an armed robbery or attack is likely and civilians
carrying guns can be common place in some areas. If, however you do get involved in an
incident when someone is armed please remember the following basic points:

Don’t panic, remain calm(One or two deep breaths may help)

Co-operate; do not resist.
o
Assume he is armed
Don’t engage in unnecessary conversation- this can be misinterpreted
o
Don’t hesitate or resist handling over personal valuables (Watch, wallet etc) or
o
Company property.
o Do not make sudden movements.
o In case of car hijacking, don’t demobilize your vehicle- If leaving your vehicle try &
stay to the side of it; do not go to the front or behind the vehicle.
o Do no make direct eye contact. They may react it they suspect that you are
preparing a mental picture of how to describe them to the police.
o Alert your immediate supervisor and police, if situation permits, as soon as possible
after the incident.
Remember: You CAN replace stolen items, but NOT your life.

Appendix 1. Emergency Response Plan

1- Introduction

The following section details about Emergency Response arrangement used in Dalma
Energy to meet the company’s HSE Policy

1.1 Emergency Response Management

To achieve Dalma Energy Emergency Response HSE management objectives Rig specific
emergency response plan is developed and issued for work location.

1.1.1 Emergency Response Philosophy

The Dalma Energy Emergency Response Philosophy is to ensure that emergency


response activities are carried out with the following priorities:
 Safeguard Lives People
 Protect the Environment Environment
 Protect Company or Third Party assets Assets
 Maintain the Company Image/Reputation Reputation

Personnel involved in dealing with the emergency shall follow these priorities when making
decisions and formulating an action plan.
At no time should any action be taken that could endanger the lives on personnel.

HSE Management objectives with regards to Emergency response philosophy is to ensure


prompt and correct response of Dalma Energy Staff to each type of emergency in order to
minimise harm and loss to personnel, environment and company assets and to resume
normal operations as soon as possible.

1.1.2 Emergency Response Analysis


Escape, Evacuation and Rescue Analysis is part of Emergency Response process for all
potential major accident events and it can be summarised as follows:
 Escape from the danger zone, and
 Muster in a safe area, and

 If you observe or in contact with any emergency you should dial the number:

 If the emergency is in the coast the LETO will ask you six basic questions in order
to have details of the incident and to know your immediate requirement – Fire
Brigade and/or Medics.
 LETO is empowered to mobilise either/both response services – Fire Brigade
and Medics if required.
 The ETO will then contact the Local Emergency Base Controller (LEBT) and the
entire CLIENT’s Emergency Response Management System (ER-MS) is thereby
activated.
Information to be given includes:
 Name and position of person reporting the emergency and contact number

 Brief description of the emergency

1.2.1 Command during Emergencies

a) On Scene Commander

CLIENT Drilling Supervisor is the On-Scene Commander who manages the


emergency response process.
The On Scene Commander will report directly to the LEBT and systematically keep
him updated with developments. Below are the generic responsibilities for the OSC,
these should be applied to all emergencies which are scenario dependent.
Priorities for the OSC are:
1. Accounting for personnel
2. Ensuring timely Search and Rescue while not placing emergency services at
risk
3. Protecting the environment
4. Preventing escalation of the emergency by ensuring stabilization or close-in of
the well
Ensure clear leadership is established quickly. Let all personnel know the current
situation and the course of action
It is essential that all personnel REMAIN CALM - It is the role of the OSC and EC to
actively manage stress and potential panic, both in themselves and others.
On an ongoing basis, the ON-SCENE COMMANDER will:
Gather information from personal observation, Emergency Controller, Command and
Control board, other team members and reassess emergency response; informing
the LECT of:

b) Emergency Controller

The Emergency Controller is the person who controls the activities of all of the
emergency teams on-site and confirms the well/process/utility shutdown.
He reports and liaises with the OSC, working closely as a team, both informing and
requesting external support/assistance as required. His generic responsibilities are
 Proceed to Command & Control Point; Register as "Mustered"
 Put on the EC's Jacket (Fluorescent Jacket)
 Receive briefing on situation including i) What has happened; ii) Current well
status iii) Personnel known/suspected to be involved; iv) Notifications made
 Mobilise Camp Medic if not on site
 Organise Fire Team and delegate actions as specified by OSC (process / well
shutdowns, initial fire fighting, search and rescue)
 Organise medical team / first aiders and delegate actions as specified by OSC
 Confirm process / well shutdown
 On arrival - advise CLIENT Fire Team (Fire brigade) of facility status (missing,
H2S, potential for escalation)
 On arrival – advise CLIENT medical team on injured status, no. missing
 On arrival – advise earthmoving equipment, cranes etc of actions as authorised
by the OSC
 On arrival – advise police requirements to limit access, block roads,
inform public
The above generic responsibilities should be applied to all emergencies

Emergency Controller is also responsible for coordination of:


 Process shutdown and isolations
 Fire teams, medical, security and resources

c) The Muster Controller


The Muster Controller is responsible for the collection of personnel status
information from all parties to identify:

e) The Fire Team Leader


The Fire Team Leader is responsible for:
Fire team status / progress / findings
Medical status of casualties / Medic activities
Operations status of Rig

Medic will perform following duties
Attend to injured
Assess if injured can be stabilised at camp clinic
Request additional medical support
Inform OSC/EC if aerial medevac will be required
Prepare injured for travel if required
Liaise with CLIENT / Medical Support on arrival

1.2.2 External Emergency Response Support

All Emergencies are normally handled through CLIENT’s Emergency Crisis Centres
 Interior : Local –Emergency Crisis Team Centre (LECTC) at Hassi Messaoud
 Corporate: Emergency Co-ordination Team (CECTC) at Algiers

Call outs for local CLIENT Fire Services & Local CLIENT Medical Services (including
MEDEVAC) is done by LETO if requested by OSC.
CLIENT Well Engineering performs technical support role to LECTC and CECTC
The decision to mobilize requirement of resources outside of CLIENT Emergency Services
and resources is taken by Client’s Corporate Emergency Co-ordination Team at Hassi
Messaoud.

1.2.3 Communications
The communication systems deployed on LR 12/LR13 comprise of Intercoms, 2-way
handheld radios (Walkie Talkies), External Telephone links and, HF, VHF radios, Public
address system & Emergency Siren.

Intercom 2-way Doghouse to TP Office, Rig floor, Mud Tanks,


shale shaker tank, Monkey Board and maintenance
office; Telephone type, Make-Gaitronics.

1.3.3 Rig ER Inductions

All new employees joining the company are required to undergo induction. The induction
takes place in three stages. During first stage, employee induction is carried at Hassi
Messaoud Base (Part-I). Induction is continued on the rig as Part-II & III.
Dalma Energy’s Training/Medical Examination Programme is structured into 4 categories
and detailed in the following HSE MS Procedures:
- Employee induction programme (Section 2.6)
- Medical Facilities & Medical examination (Section 2.15)

1.4 DETAILS OF EVACUATION AND ESCAPE EQUIPMENT

1.4.1 Evacuation and Escape Systems

Evacuation and Escape arrangements with reference to emergency plans are described in
following Safety System Procedures
- H2S Precaution & Response
- Emergency Response on Land Rigs (Section 2.12)
Evacuation and Escape arrangements for H2S response is reproduced below as an
example.
 On hearing of emergency alarm the driller shall activate the air horn & don the
escape set together with his crew.
 The driller shall secure the well with the assistance of Drill crew as per
standard well shut- in procedure.
 If the Derrick man is up on the monkey board, then he shall don the escape
set, climb down the derrick & join the drill crew.
 After the well is safely secured, the drill crew shall proceed in an upwind or
crosswind direction to the upwind H2S cabin.

 The driller shall note H2S reading from the H2S monitoring unit before moving
to the upwind H2S cabin with the crew.
 Mud tester shall down escape set, secure the worksite & move to the upwind
H2S cabin. Any other staff present shall move to muster point without delay on
hearing the alarm.
 All other personal on the rig site shall proceed to the upwind H2S cabin,
dedicated escape set wears after having donned their escape set & secured
their work place.
 The gate shall be locked by the Gate guard & the control board shall be taken
to the secondary assembly point if the wind direction requires the crew to
assembly there.
 The Rig clerk (Radio Operator) shall use the control board to make a head
count of the personnel at the assembly point.

Search for missing person in rig site:

If any person is found missing then the following actions shall be taken
 As immediate response two search team A & B & a back up team shall be
organised. Each team shall consist of 2 persons who shall wear 30 minutes
breathing apparatus. The senior member of a search team shall be the search
team leader of that particular team. He shall be equipped with the respective
search route plan & conduct the search according to plan.
 If any of rig personnel is sleeping in rig accommodation not being aware of the
situation, he shall be rescued by the search team & brought to upwind
assembly point after he has donned his escape set
 If H2S victim is located, the search team will immediately move the victim to
the safe assembly point for first aid & immediate action will be taken by Medic
to review casualty with 100% Oxygen treatment using Oxygen Resuscitator,
then casualty shall be rushed to the nearest clinic for expert medical care.
 If no victim is found, the search team shall go to the assembly point and wait
further instruction.
 The back up team will remain on stand by the assembly point for providing
any assistance to the 2 search teams if required.
 The Tool pusher at the assembly Point shall designate 2 reliable crew
members to don 30 minute B.A. sets & send them to survey the location with
portable H2S detectors. On completion of survey they will return to the
assembly point & report to the Tool pusher.

 The Tool pusher will discuss the findings with client representative on site &
inform all parties of the situation on hand including local emergency base
team & wait for further instruction.
 Only after “All clear” is given by LEBT work is to be resumed as normal.

1.4.2 Means of Recovery to a Place of Safety

Arrangements for rescuing personnel from Rig and transporting to a place of safety are
as follows:

a) Stretchers
Two types of stretchers are available on the rig. One is a manual carry type
(Make: DOHERTY), while the other type (Make: LECSAFE) has a provision for
lifting by crane. The stretchers are stored one in each Safe Breathing Area (SBA)
Cabin, one in the dog house and one in the clinic
Additionally each Rig has been provided with a Scoop Stretcher to be used for
lifting casualties who must not be moved unduly, or who must be lifted in the
position in which they are found (for example, a casualty with spinal injuries). This
stretcher splits into two halves, which are placed on either side of the casualty,
then reassembled

b) Breathing Apparatus
SCBA: 6 sets in the Primary H2S Cabin, 6 sets in the Secondary H2S Cabin, 6
cylinders in the Primary H2S Cabin, 6 cylinders in the Secondary H2S Cabin, 04
sets in Dog house, 1 set in Maintenance Office, 1 set each in Dalma Energy-
TP’s Office, Client DS’s office, client Safety representative’s office, Geologist’s
office and OLC’s office,
Escape Set : 2 sets in Dog House, 1 set on Monkey Board, 1 set on Active Pits,
1 set on shale shaker area, at least 4 cylinders in the Primary H 2S Cabin, at least
4 cylinders in the Secondary H2S Cabin).

c) Assembly Points and Search Routes


Assembly points are located at the primary and secondary H2S cabins and are
prominently signposted. The search routes for rescue are posted inside both of
these cabins.

d) First Aid Equipment


First aid kits are located at the Assembly Points, Dog House, TP’s and
Maintenance office on the rig site and the messes at the camp.

The clinic located at the camp site the list of equipment shall be approved by
CLIENT. The clinic shall be marked for identification as defined in Dalma Energy
sign Standard.

f) Ambulance Services
A 4WD vehicle capable of carrying one stretcher shall be provided.
 Vehicles must be on the location at all times (at the rig or camp site, but should
be in close proximity to the rig medic and crash box).
 The list of equipment contained the ambulance shall be approved by CLIENT.

1.5 Emergency response Contingency Plans

1.5.1 Blow Out

A blowout is defined as an "uncontrolled flow of gas, oil or other well fluids into the
atmosphere". A blow-out situation will not only affect the facility and will involve CLIENT
corporate response as well as drilling and service contractor personnel.
In a blowout emergency the primary objective is the safeguarding of human life. Secondary
objectives are minimising damage to the environment, assets and protection of CLIENT
reputation.
The Local Emergency Coordination Team (LECT) has responsibility for the emergency in
conjunction with the OSC until the safety and proper treatment of all personnel at the
location has been ensured and escalation has stopped.
Once the objectives of safeguarding life in complete, and escalation has stopped, the
blowout moves into recovery phase, see Blowout Contingency Plan. This section does not
address routine well control procedures as these are contained in DOPs

1.5.2 Firing the well

Setting the well on fire should be considered if human life is threatened or there is a risk of
an uncontrolled explosion.
Benefits of firing the well are no uncontrolled explosion and a reduction of environmental
damage. In addition, well control specialists tend to prefer to work with a well on fire.
Consideration should be given to explosion and radiation patterns on the surrounding area.
Firing the well can be done provided the following guidelines are adhered to with
authorisation of the LEBT:
1. Use a rocket flare or equivalent with at least 150m range. (Obtainable via the
LEBT).
2. Select a spot with good cover upwind of the Rig and easy route for retreat.
3. Two men should approach the spot wearing SCBA. One must carry Combustible
Gas Detector.

1.5.3 Rig Failure


A rig failure is a rig emergency where one or more of the principal functions of the rig are
inoperable in combination with a hydrocarbon bearing formation that is open in the well.
Examples of rig failures include Substructure collapse, Mast failure, Foundation collapse,
Drawworks breakdown, Mud pump failure, Power failure

with cement or barite, if so put on standby
If the well cannot be shut in assess whether of not the rig is safe to approach. If not
cordon off area (hazard tape, drums etc), go to Recovery.
If there is risk of Hydrocarbon leak, mobilise equipment for environmental protection

If the well can be shut in and there is no risk from hydrocarbons, mobilise required
equipment and resources for repair to site.

1.5.4 Fire/Explosion

This section addresses explosions and fires that may involve facilities or equipment items,
that has risk to personnel and has a risk of fire spreading.
Primary fire-fighting must not endanger personnel. The first priority in any emergency is the
safety of personnel. Primary fire-fighting may be carried out by trained personnel using the
appropriate type of fire extinguisher. If no fire team is available on-site, they the primary
response must be by the CLIENT fire-team on arrival.
If a fire breaks out at the associated facility camp, the OSC must mobilise to the camp.

On Scene Commander

The primary concern is for the safety of Personnel and secondary concern is for the
Environment, the Facility and associated equipment will come third

Ensure the BOP is closed and the well is secure (for fire on site)
Review the emergency, provide an update to the LEBT including:
− Personnel accounting /status

that may fuel the fire or explode
If unable to move flammable object, consider placing solid object i.e. skip, open

If fire looks like it spreading to adjacent buildings or equipment, provide cooling

The Competent Person is one who is trained specifically in the hazards and use of Nitrogen
by his company.
As the majority of Nitrogen used on locations is under the control of contracted companies,
the competent person is therefore the Nitrogen Operator. He is responsible for
transportation, storage and handling of the Nitrogen. In the event of a Nitrogen related
emergency he is required to implement his own company Emergency Procedures/Actions.
He must provide and advise the OSC with all relevant information with regards to the
equipment, inventory and course of action.

1.5.6 Gas leak

Gas leaks may arise from gas lift lines to nearby wells or from gas used in the process of
under-balanced drilling.
A gas alert may be activated by
 Observation
 Fixed or Portable Gas Detection System

o Suspected Gas Leak

The immediate requirement is to confirm the leak. Once the real possibility of a gas leak
is verified, response must be as per CONFIRMED GAS LEAK.

On Scene Commander
Action Check
For a Manual Report - If a person reports/suspects a gas leak the
immediate need is to confirm whether there is a gas leak or not
Send an operator with a combustible gas detector to confirm the source
of the reported leak. Instruct Operator to return IMMEDIATELY on any
positive indication of gas as he approaches the affected area - 10% LEL
at the edge of the area could indicate 100% LEL at the source
For a portable combustible gas detector alarm: For low gas (>20% LEL)
detection - Confirm with 2nd combustible gas detector

locations to alert increasing gas levels
Isolate public access routes downwind of the site
Fire Teams will not be sent to the source of any major gas leak, but they
may be used WHERE SAFE TO DO SO to:
Search for missing persons in safe areas
Reduce chances of escalation - cooling, manual isolations, cloud dispersion
The On Scene Commander must define the limits of any Fire Team
deployment. He must check:
 If the shutdown system is working and the line is blowing down
 Is the release contained or dispersing safely

1.5.7 H2S

At sites where H2S is recognised as a possibility (Level 3), sites are provided with:
 A Primary and Secondary Muster Point at opposite ends of the site
 Windsocks to show prevailing wind direction
 H2S Detectors that actuate a continuous H2S alarm or which actuate the General
Alarm
 H2S Cascade System
Site personnel are instructed that on either H2S Alarm (or General Alarm if no dedicated
H2S Alarm is available) to observe the wind direction and muster at the upwind Muster
Point.
There is no auto-shutdown of facilities upon H2S alarm, however the SCBA allows for
crews to rig up with oxygen masks and work (shutdown) in the area where H2S is present.

In particular he has to assess the likelihood of an explosion and the risk to the
surrounding area and the public.

Clear all non-essential personnel upwind well away from the site

A Search and Rescue of missing personnel may be required once personnel accounting is
complete and either personnel are missing or are identified as being trapped in a
hazardous situation.
On Scene Commander
The On Scene Commander must assess the scale and the potential for escalation.
Any Search and Rescue must not endanger search teams.

Establish number of personnel missing
Try to establish last known location of missing personnel through interview of
other crew members
Confirm whether the emergency is escalating or is under control (all fires are out,
BOP's closed and well secure etc)
Confirm that all ignition sources are isolated
Brief search teams [Fire teams and Rig crew] on situation including:
 Any residual risks
 Actions to take on identifying further problems (sight, sound, smell)
Advise to WITHDRAW IMMEDIATELY if there is any doubt as to safety of team
 Number of personnel missing and expected location
 Safe exit routes
 Special equipment required, SCBA, portable combustible gas detectors etc
Get authorisation from LEBT for Search and Rescue to proceed
Ensure that radio contact is maintained with search team at all times
Observe if possible the area being searched
Monitor conditions and detection alarms
Advise medical services of safe location to set up triage area

In any emergency there is potential for casualties. If any casualties or potential for
casualties the Emergency Services must be mobilised as quickly as possible. The role of the
Medic in the field is to perform triage and casualty stabilization, but if facilities are not
adequate for a serious injury or multiple casualties, the LEBT must be informed as quickly as
possible for the above so that he can organise CLIENT (Area and Corporate) medical
support and/or aerial Medevac.

On Scene Commander
The On Scene Commander must assess the scale and the potential for escalation. In
particular he has to assess the likelihood of an explosion and the risk to the
surrounding area and the public.
Action Check

IF DRILLER IS INJURED, SECURE THE WELL


Inform ETO / LEBT of medical emergency and confirm medical support is
mobilised to site. Advise if medevac is likely to be required
Assess if dangerous situation still exists, take steps to clear danger
Advise Medic / CLIENT medical services of safe location to set up triage area
Liase with medical staff as to determine the extent of the injuries, inform LECT
Find out the number of casualties that require evacuation and type of evacuation
i.e. air, ambulance or available ‘on site’ transport
Ensure that any injured persons that are evacuated are name-tagged and have
details of injuries, treatment given and any other information available
Keep a record of casualties evacuated and if known, to where they are evacuated
If there has been a fatality then:
 If possible ensure that the incident scene is isolated / not disturbed
 The ROP should be informed (by LEBT) and be accompanied to the incident
site by a CLIENT Representative
 The incident area should be sealed off as soon as emergency services have
finished

1.5.10 Environment Spill

On Scene Commander
The On Scene Commander must assess the scale and the potential for escalation. In

IF SPILL IS IN THE VICINITY OF THE DRILLING FACILITY, ENSURE THE


WELL IS SECURE AND MUSTER PERSONNEL
Ensure that all personnel are accounted for and have been remove to a safe area
upwind of the facility
Position personnel on access routes to stop traffic entering area
Mobilise emergency services to the site
Control measures only to be carried out after taking the required precautions for toxic or
combustible gases, i.e. gas tests made with breathing apparatus
Remove any ignition sources / establish ignition free zone (demarcate using hazard
warning tape / drums etc)
Assess the Spill and inform LEBT of:
 Substance spilled
 Potential for / or presence of poisonous and explosive gases
 Identification of possible ignition sources that could lead to fire or explosion
 Identification of the source of the spill
 Approximate volume of the spill
 Is the spill being contained or not
 Is there access for spill containment equipment and vehicles
 The environmental sensitivity of the area (Flatness of the area, Distance to a
Wadi, Distance to potable water source, Distance to settlements, Distance
to nature reserves (like Oryx Area), Permeability of surface rock)
 Any adjacent facilities or settlements which may be threatened

Request resources from LECT these may include vacuum trucks, pumps, dirty
crude tanks, transporters, earth moving machinery, loaders, dump trucks, piping,
barriers, telecoms skid

An explosive emergency has occurred if an unplanned detonation has taken place on the
surface (see Explosion) and un-exploded material remains on the site.
Objectives of this contingency after ensuring personnel are safe is to ensure remaining explosives
are in a safe condition. Material covering hazard assessment, controls and recovery concerning
Explosives can be found in the Hazard Register.

Competent Person
The Competent Person is one who is certified to work with Explosive Devices used by his
company.
As the majority of Explosive Devices used on the Rig belong to Electric Logging Contractors
the Competent Person is therefore the Logging Engineer. Should Tubing Conveyed
Perforating guns be used the Competent Person is then the Perforating Engineer.
The Competent Person is solely responsible for transportation, storage and handling of the
Explosives. In the event of an Explosives Incident he is also required to implement his own
company Emergency Procedures. He will also provide the OSC with all relevant

information on the Explosives. The Logging Company operators are not normally
Competent Persons.

The Incident Scene must not be approached until given the all clear by a Competent
Person. As the effect of certain types of Explosives can be felt over some distance ensure
there is solid cover between the Explosives and all personnel. After clearing the area
contact the LECT will mobilise a Competent Person.
No attempt to recover or handle Explosive Devices should be made by any Rig Personnel.

1.5.12 Radiation Emergency

Chemical radioactive sources are used in wireline electric logging tools. Radiation can also
occur naturally and is known as NORM (Naturally Occurring Radio-active Material).
Potential for NORM is assessed prior to well handover and is not considered in this
procedure (see SP-1170 NORM). A radiation emergency has occurred if:
 Radiation source is lost on the site
 A radiation release occurs outside the control of planned operations

In the event of a Logging Tool containing a Radioactive Source being stuck downhole, refer
to CLIENT Drilling Operations Manual (check with CLIENT)
Additional material covering hazard assessment, controls and recovery concerning
Radioactive Sources can be found in the Hazard Register,

1.8.12.1 Radiation Released

Any additional equipment or specialist manpower required will be organised by the


company responsible for the source, CLIENT have no internal resources available for
handling Radiation Incidents although TKL/2B - the CLIENT Radiation Protection
Advisor has training for a Radiation Incident

All personnel receiving close to or over the considered safe weekly dose will be
required to take a Medical Examination. Anyone suspected of receiving a Radiation
Dose above the safe limit should be immediately evacuated to the coast for medical
examination
Inform LECT of requirement for medevac
In Radiation Incidents a thorough assessment of all personnel exposed to the
radiation is required
Provide list of names with the level of radiation that they have been exposed to
and the length of time they received that dose

Competent Person
The Competent Person is one who is trained and certified to work with the Radioactive
Sources used by his company. He is also aware of Local Regulations concerning use of
Sources.

As the majority of radioactive sources used on the rig belong to the electric logging and
MWD/LWD contractors, the competent person is therefore the logging engineer. He is
responsible for transportation, storage and handling of the sources. In the event of a
radiation emergency he is required to implement his own company Emergency Procedures.
He must provide the OSC with all relevant information on the source.

1. Defined Command and Control Station (recommended at Main Gate). Command and

For sites with less than 20 people normally on site, then a reduced team is acceptable
however the OSC and Intervention Team Leader Must be defined for all sites.

3. Two defined Muster Points (Primary and Secondary). For known H2S sites, windsocks
must be installed.

At Main Offices (Coast)


It is a Client’s requirement that Dalma Energy & Co. LLC handle the NOK of our own
employees where the employee is a casualty of the emergency. In order to facilitate
information flow between the Dalma Energy Company and the emergency response, it may
be necessary for a Contractor Senior Management Representative to join the CECT

The following is a guideline of DEC requirements:


Duty Emergency Person: This person will be contacted during the Emergency by the CECT.
He may be asked to join the CECT and be required to contribute to the Emergency
Response and Recovery processes.

Human Resources Staff: These staff are to direct media calls to CLIENT during an
emergency. They are also required to facilitate the informing of next of kin once a Dalma
Energy casualty is confirmed.
