Dear Mr. Henary,

I am John working as a construction manager in the execution department in your company. I requesting
you to grant me one month's leave for my father's post his knee replacement surgery in November
month from 15th November to 14th December when my current project will approximately complete and
there will be a minimum of two month to start my new project.

My father is suffering from arthritis ailment and the doctor recommends a knee replacement surgery to
rid from the wheelchair. After surgery, there will be four weeks of rehabilitation where I will have taken
to him to a physiotherapist every day for a physiotherapist session.

As the project will be in the completion stage, there is no so much workload. I have convinced my junior
to take responsibility and accountability in my absence. He will handle my role in my absence if there
will be a need for me. He is capable to handle my role easily. I will available on the phone in
emergencies and give instructions on the phone. When I will away from the office, I will be promptly
responding through emails and read the daily status report to keep updated myself with day's work and
project roadmap.
