RE: Guidelines on the Documentation of Student Activities

Good day.
With the transitions of almost all activities conducted online and abiding the protocol to
be in implemented in an online manner, please be guided of the following:
1. Final Lists of Officers (Appointed and Elected)
2. Organization Adviser/s
i. Inform the adviser/s to send email to dexterpauldioso@uno-r.edu.ph with the
following text: “I, ______name___________, accept to be the adviser of
______________name of the organization_______________”
3. Approval of activities shall follow the “Activity Form Online” format and procedure
and shall be passed to the same email above.

In behalf of student affairs and student welfare, we extend our gratitude for providing
activities to students online. Moreover, to properly account and document our activities, all
activities shall be on hold effective September 18, until all requirements are complied and the
office advises for the resumption of activities per organization.
