Induction Program Clear Credential Requirements

A signature below indicates that the teacher has been advised of the LACOE Induction program requirements. Local districts may charge a fee for providing
support for teacher participation. Any extension of the program requirements requires payment for the services at the current year program and district rates. See
promissory note for current rate. The following is a summary of the Candidate and LACOE Induction Program agreement.

The Candidate agrees to:


1. Attend program advisement and 2 Teacher Learning Community meetings. (in person or virtual meetings) ________

2. Candidates and Mentors are expected to meet weekly for at least 4 hours per month. ________

3. Complete a Candidate Log to record weekly meeting with Mentor after each meeting. ________

4. Examine the California Standards for the Teaching Profession (CSTP) in the context of the content area of focus. The Core activities are
designed to assist the Candidate and Mentor in the development of an Individual Learning Plan (ILP). This process establishes the foci of
each Module. ________

5. Complete Evidence of Teacher Practice assignments with Mentor during weekly meetings within each module in the following areas:
a. Collaboration; Reflection; Context; Professional Development ________

6. Upload all components of each module as described in the Online Learning Management System (Canvas) as they are completed.

7. Compile a portfolio within Canvas that demonstrates
a. knowledge and application beyond what was demonstrated for the preliminary credential, and
b.growth and competency in each of the six CSTPs.
(Note: In some cases, additional time beyond the two-year program may be needed to show Candidate competency for clear
credential recommendation.) ________

8. Complete local program evaluations/surveys. ________

9. Use Canvas and the LACOE web site to access resources, documents, and contact information. ________

10. Communicate with program staff promptly to:
a. update contact/employment information, program status, or extension of deadlines
b. respond to emails, phone calls, verbal and written program requests
c. identify support issues (Request for Assistance Form available through Canvas/Pages)
d. share change in program status (Change in Program Status form available through Canvas/Pages) ________

11. Always conduct themselves with professional integrity. It is essential that Candidates model proper and legal aspects of professional
integrity. Failure to demonstrate professional integrity may result in removal from the program. ________

12. Make monthly payments as stated on the signed Promissory Note. (Note: program will be placed on hold if payments are not current;
unenrollment may occur if payments are behind schedule) ________

13. Acknowledge being provided information about the Early Completion Option (ECO) - requirements include a minimum of two years
positive teaching experience documented by positive evaluations from the prior two school years, as well as the approval of the current
site administrator - Application deadline: October 15th. ________

14. Acknowledge receipt of the Beginning Teacher Program Induction Handbook and understand that it is my responsibility to read and
comply with the policies and procedures contained in this handbook and any revisions made to it. ________
