AFFIDAVIT

I, Mr DING SALAN, Filipino, of legal age, single and presently residing at 123 Mabolo Drive, Ermita,
Manila, after having been sworn in accordance with law, do hereby depose and state that :

1 I am a liaison officer at LUKBAN, MENDOZA, MONTER, NG, PORCIUNCULA & ROXAS Law
Office with office address at Unit 103, Times Square Plaza, UN Avenue cor Taft Avenue,
Manila;
2 That in the morning of Sept 26, 2017, I was instructed by Atty Lukban, a senior partner, to
serve the NOTICE TO VACATE to Ms. Ana De Castro at her residence at No. 35 Gagalangin,
Tondo Manila;
3 That at the aforementioned address, I was able to speak with Ms. De Castro and told her
that I am delivering a Notice to Vacate to her;
4 That the said Notice to Vacate was duly received by Ms. De Castro. A copy the said notice is
hereto attached as Annex “A”.

IN WITNESS WHEREOF, I have hereunto affixed my signature this 26th day of September 2017 in
