GUIDELINES FOR
ENROLLMENT
Instructions:

1. Accomplish and sign the Confirmation & Undertaking

Form and the Personal Data Sheet and send to:
law.admission@ust.edu.ph

 Please indicate the email Subject as:

“For Enrollment BATCH 4 – [Surname, First
Name]”

2. For Non- UST Graduates, please submit the following

documents through courier:

a. Official Copy of Official Transcript of Records; and

b. Honorable Dismissal

addressed to:

Attention: Ms. Vicky Ladisla- For Civil Law Application

and/or
MR. CESAR VELASCO
Office of the Registrar
2nd Floor, Main Bldg.
University of Santo Tomas,
España, Manila
If you are unable to send the transfer credentials
forms/documents at this time, please accomplish and sign the
attached “conditional admission contract”.

3. For UST Graduates, you will be required to submit the

original copy of Official Transcript of Records and other
requirements at a later time (but you should now apply for
these requirements in the appropriate UST offices).

4. Upon submission of the required documents, you will be

notified if your name has been encoded in our roster. As
soon as you receive the notice, you can proceed with online
payment.

5. You may also be required to accomplish other University

Forms in the course of your enrollment, as may be sent by
email.
