I am writing to request permission to conduct a research study at your office (BJMP Regional Office I). I
am currently enrolled in the Master’s in Information Technology at Don Mariano Marcos Memorial
State University and am in the process of writing my capstone project. The study is entitled Human
Resource Information System of Bureau of Jail Management and Penology Region 1. No costs will be
incurred by either your office or the individual participants.

I am hereby seeking your consent to gather necessary data on Human Resource Information System on
your office and approach employees to provide participation for this study.
