CHAPTER II

COMPREHENSIVE NURSING HEALTH HISTORY

Client RTS was born on October 30, 1992 and is currently 27 years of age. She lives
in San Isidro, Babatngon, Leyte and currently not employed. She is a high school graduate.
She is currently a single parent and still living her parents.

Before pregnancy the client had her menarch when she was 14 years old. She has a
regular menstruation every month with duration of 3-5 days. The client also stated that she
does not experiences dysmenorrhea during her period.

The client’s pregnancy history is primigravida and primipara. Her last menstruation was
on February 28, 2019. The client stated that her pregnancy was not planned. She gave birth
to her first child on December 4, 2019, at RHNC-EVRMC through a cesarean section.

FAMILY HISTORY

The client’s father and mother along with her six siblings are alive and healthy. She stated
that none of the have any illnesses nor are taking some medications. Their type of family is
Nuclear.
