AMENDED AFFIDAVIT

I, SENY ERPILO KUMAZAWA, of legal age, married and

resident of Juan Luna St., Lamacan, Argao, Cebu,, after having been
sworn to in accordance with law, do hereby depose and say:
1. That I am the respondent in the Civil Case No. CEB-33269,
Branch 24 of the Regional Trial Court of Cebu City;

2. That I was married to KAZUTOSHI KUMAZAWA on

November 3, 1998 at Caloocan City, Metro Manila, Philippines;

3. That after the marriage KAZUTOSHI KUMAZAWA and I

begot two Children, namely KAZUMI REIKA and KAZUKI SEIJI;

4. That previously, the Honorable Court in the above-captioned

case granted my motion for support in the sum of P8,000.00 for each
child or a total of P16,000.00 per month;

5. That more than four (4) years have elapsed and that
KAZUMI REIKA and KAZUKI SEIJI is now twelve (12) years old and
KAZUKI SEIJI is now ten (10) years old;

6. That lately, I am experiencing financial difficulties of the

rising cost of living, educational expenses of the two (2) children and
due to the medical condition of KAZUMI REIKA;

7. That lately, KAZUMI REIKA has been coming in and out of

Hospital because she is suffering from SEVERE ASTHMA;

8. That attached hereto are the receipts, prescriptions and other

documents as proof that indeed KAZUMI REIKA is suffering from
SEVERE ASTHMA;

9. That aside from SEVERE ASTHMA, KAZUMI REIKA’s

hearing is impaired and that I am spending around P1,200.00 every
two (2) weeks;

10. That with this predicament, I am left with no other choice

but to come to this Honorable Court so that the support may be

increased from P8,000.00 per child per month to P20,000.00 per child
per month;

11. That I am now saddled with debts and that that there is
nobody else that I can turn to except this Honorable Court;

12. That petitioner KAZUTOSHI KUMAZAWA has sufficient

property and money to comply with the increase of support should
the Honorable Court grant my motion for increase of support.

13. That I execute this affidavit to attest to the truth of the

foregoing and for whatever purpose it may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 13 th
