JUDICIAL AFFIDAVIT

SWORN STATEMENT in question and answer form of GREGGORY H.

WHITE before Atty. Rex B. Store, Jr.

I, Atty. Rex B. Store, Jr. of legal age, married, Filipino, a member of the
Philippine Bar with office address at No.42 Ilang-ilang St., Lucena City, and
whose name appears in the Roll of Attorneys as No. 69696, hereby conduct
this examination of witness in my office.

ATTY. REX B. STORE Jr,: Do you swear to tell the truth, the whole truth and
nothing but the truth?
GREGGORY H. WHITE: Yes, I do.

Q: State your name, status, and address.

A: I am Greggory H. White, married to Skyler White, and a resident of Brgy.
Tibay, Lucena City.

Q: Are you fully aware that you’re answering these questions under oath and
that a perjury case may file against you if you lie in this testimony?
A: Yes, I do.

Q: Do you swear that all information and knowledge you will state today is
based on truth?
A: Yes, I do.

Purpose: The purpose of this witness is to prove that he is the husband of the
accused, and that his wife is not guilty of anything charged against her, that
they were together at the time of arrest of the wife in Oct. 17, 2019 while they
were on their home and not committing any crime; and that she reported the
same to the police blotter and to deny any allegations against her.

Q1: Mr. White, what does your wife do for a living?

A: She manages our carwash, sir.
Q2: What is her nickname?
A: She is also known as Sky, sir.

Q3: Where were you on Oct. 17, 2018?

A: On that afternoon sir, my wife said she’ll be buying more pasta in SM
because it was our son’s second birthday party.

Q4: Did she immediately return after that?

A: No sir, I’m very worried because it was almost midnight and she still hasn’t
returned.

Q5: Do you know what vehicle did she use to go to SM?

A: Yes sir, she boarded on JumJum’s trike.

Q6: Did you contact JumJum?

A: Yes sir, he said he fetched another passenger after my wife alighted outside
SM.

Q7: What did you do after that?

A: I contacted my wife in her phone number sir.

Q8: Did she reply?

A: No sir.

Q9: Then what did you do?

A: I reported it in barangay sir and in the police station.

Q10: At following day, Oct. 18, 2018, what actions have you taken?
A: I went to the General Makoy police Camp in Lucena City to inquire sir. But
to no avail, there were no reports nor incident involving my wife sir.

Q11: After that what did you do?

A: I insisted that they I want to have a police blotter for my wife but they were
non-compliant, because according to them, she is in her right age and has no
incapacity. But after my insistence for several hours, they adhered and agreed
for the blotter

Q12: Is there an evidence that you really executed a blotter?

A: Yes sir. This one. (Marked as Exhibit 1)

Q13: After the blotter, did you follow up for the whereabouts of your wife?
A: Yes sir, in the same police station, I asked them if there is new information
about my wife.

Q14: What did they say?

A: They said they arrested my wife around 3:00pm of Oct. 17, 2018 together
with three men and women, that’s why I asked where are they detained.
Q15: Did you ask where she was detained?
A: Yes sir, they said in she was in Brgy. Payapa lock-up jail.

Q16: Did you immediately went to the said lock-up jail?

A: Yes sir, but unfortunately, she was still nowhere to be found.

Q17: What did you do afterwards?

A: I went back to the police station and asked who can I contact to reach the
lock-up jail.

Q18: Did you follow up in lock-up jail?

A: Yes sir, and they told me she was indeed there but it was 11pm of October
18, 2018.

Q19: Is there a complaint filed before the court against your wife at that time?
A: None sir, even in barangays.

Q20: After she posted bail, and went out did she say something to you about
the arrest?
A: Yes sir, she said that she was arrested on the afternoon of Oct. 17 when she
is about to buy food for our son’s birthday and that her phone was confiscated
that’s why I was not able to reach her during those times.

Q21: Did the police really found illegal drugs in her possession?
A: She said the police got none. She was just detained because the real persons
to be arrested were the ones beside her, two man and a woman. She was so
confused when they brought her with them.

Q22: Would like to say something?

A: Yes sir, I am just confused as to why they didn’t let my wife call me or
why’d they confiscate her phone and bar her to call anyone during said arrest.
Me and my family were so worried about my wife.

Q23: Are you willing to sign and attest that this document is indeed executed
by you under oath?
A: Yes, sir.

IN WITHNESS WHEREOF, I have hereunto affixed my signature this

th
20 day of October 2018 in Lucena City.
