Reminder: This is your official class list; do not accept students who are not enlisted here. Your schedule and room stated above are the official room and schedule where
you should conduct your class.
Notes:
1. The faculty should secure the tentative class list from the College Secretaries on the first day of class.
2. Faculty are advised to secure the final copy of this Class List at least a month after the start of classes.
3. Teachers are NOT allowed to accept students if they are NOT included in this list.
4. Students who are included in this list but are not attending classes for three meetings should be reported by the subject teacher to the Guidance
Office.
5. The names listed in the Official Class List are the basis of the system where faculty will encode the grades. Grades of students who have
continuously attended the class but are not included in this list will not be given grades; also, it will not be accepted by the Registrar’s Office.
Consequently, the concerned student whose name appears on the Class List/Grading Sheet of the teacher where he is supposed to be attending will
be given a failing mark by the faculty.
6. Your official room assignment and class schedule is reflected on the upper portion of this form.
7. You are not allowed to conduct classes on a different schedule or different room other than the room and schedule stated above.
8. If you would like to change your schedule or room assignment, please process your request. The request form is available at the Office of the Dean
c/o College Secretaries.
9. Transfer of room assignment or change in schedule can be made only upon the receipt of the APPROVED request form from the Registrar’s Office.
